William Henry Duncan, commonly known as Doctor Duncan, was born of Scottish parents in Liverpool. He moved to Edinburgh, where he trained to be a doctor before returning to work in Liverpool. Duncan was appointed Liverpool’s first Medical Officer of Health on 1 January 1847 under a private act (the Liverpool Sanatory Act 1846). His uncle, James Currie, was also an influential Liverpool physician.
